KID'S CAMP WAIVER
Liability Waiver & Participation Agreement – All Yall LLC
In consideration of being allowed to enter the yoga, fitness, and art area and/or participate in any activity at All Yall LLC, the undersigned, on his or her behalf, and on behalf of the participant(s) identified below, acknowledges and agrees to the following conditions:
I acknowledge that I am the parent or legal guardian of the child(ren) and voluntarily authorize my child to participate in yoga, fitness, and art activities at All Yall LLC.
I understand that there are inherent dangers associated with yoga, fitness, and art activities, and recognize that any physical activity involves risks of serious bodily injury or death, including but not limited to temporary or permanent muscular and skeletal injury and paralysis. These risks may be caused by my own actions or inactions, those of others participating in the activities, the conditions in which the activities take place, or the negligence of the participants named below. There may also be other risks not known to me or not readily foreseeable at this time. These risks may include, but are not limited to, twisting an ankle, broken bones, torn ligaments, or even death. This includes activities both inside and outside the building, as well as all surrounding areas.
Physical activities at All Yall LLC may include, but are not limited to: running, jumping, stretching, twisting, pulling, pushing, headstands, handstands, push-ups, and planks. Students may hang upside down for extended periods. Conditioning can be strenuous and demanding. Such activities often result in joint, bone, and muscle stress and fatigue, shortness of breath, sweating, and accelerated heart rate.
Knowing these risks, I and my child(ren) assume all risks, whether foreseen or unforeseen, in connection with participation in these activities. The Parent/Guardian authorizes these supervised activities.
Off-Site Activities:
Participants of All Yall LLC may engage in yoga, fitness, and art activities off-site, including but not limited to parks or neighborhood locations. Off-site events will involve the same or similar activities as referenced above. The condition of off-site areas and equipment is not known by All Yall LLC, its staff, or contractors. Though All Yall LLC will exercise due caution, we do not guarantee the safety of off-site locations and will not accept responsibility for injuries sustained during off-site activities. Parent/Guardian agrees to hold All Yall LLC, its staff, and/or contractors harmless from accidents or injuries that occur during off-site activities.
Medical Release:
I fully understand that the staff of All Yall LLC are not physicians or medical practitioners of any kind. With that in mind, I hereby authorize and give my consent to All Yall LLC, including any of its instructors, contractors, or employees, to provide emergency medical care and to grant authority to any emergency unit, hospital, or doctor to render immediate aid as may be required for the treatment of the above-named participant in the event of any emergency, either on the premises of All Yall LLC or during any activity involving the participant.
Media Release:
By attending All Yall LLC activities, whether in-studio or off-site, I hereby grant All Yall LLC the absolute right and permission to copyright, publish, and use photographic portraits, pictures, audio, and video recordings taken by any means. I grant full use of my child’s likeness, voice, and words without compensation to me or the participant(s).
Personal Property:
All Yall LLC is not responsible for lost items or for items left in waiting or changing areas.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN ALL YALL LLC AND MYSELF, AND I HAVE SIGNED IT OF MY OWN FREE WILL.
